چکیده

Non-islet cell tumor hypoglycemia (NICTH) is a rare paraneoplastic syndrome in which secretes high molecular weight (“big”) IGF-2, causing hypoglycemia. Big IGF-2 product of abnormal processing precursors with defective gene transcription and expression. Tumors involved are usually mesenchymal, vascular, or epithelial origin have burden. We report an unusual case NICTH patient concomitant spindle tumors the liver thigh. A 65-year-old male recently diagnosed malignant melanoma was referred for recurrent He noted to glucose values 30 37 mg/dL on basic metabolic panels. experiencing episodes confusion, incoherence, disorientation, required emergency department visit. During his office visit, point care 44 mg/dL. Patient denied any autonomic symptoms. Upon further evaluation, labs revealed mediated Insulin c-peptide levels were undetectable. started prednisone 20 mg daily subsequent normalization levels. CT abdomen/pelvis 15 cm left hepatic lobe. Biopsy showed neoplasm, confirmed be solitary fibrous pathology. After mass resection, he weaned off steroids resolved. surgery, thigh discovered, measuring 8x10cm, found also neoplasm biopsy. underwent excision mass. characterized by increased utilization inhibition release from due tumoral secretion incompletely processed IGF-2. Diagnosis elevated IGF-2/IGF-1 ratio our patient. Hepatic mesenchymal that remarkable size (>10 cm) most commonly identified. Mainstay treatment tumor. Alternate therapies include glucocorticoids when surgery not possible. This highlights atypical presentation setting two large neoplasms, resolution after resection larger calls attention despite there being multiple tumors, removal sized might assist

A 94-year-old man with recurrent hypoglycemia caused by non-islet cell tumor hypoglycemia (NICTH).

Non-islet cell tumor hypoglycemia (NICTH) is a rare but serious paraneoplastic syndrome in which a tumor secretes the high molecular weight form of IGF-II, causing hypoglycemia and requiring early diagnosis and management. Here, I report a rare case of NICTH caused by fibrosarcoma in a 94-year-old male with recurrent hypoglycemia. This case report describes the clinical presentation and diagnos...

BACKGROUND Non-islet cell tumor hypoglycemia (NICTH) is a newly recognized, but uncommon, paraneoplastic syndrome that is associated with tumors of mesenchymal origin. We report a case of NICTH associated with a gastrointestinal stromal tumor (GIST). CASE REPORT A 60-year-old man presented to the emergency department of our hospital after being found unconscious in his home. His serum blood glu...

Insulin and somatostatin releasing islet cell tumor caused hypoglycemia.

We report a hypoglycemic case with normal insulin levels, which was caused by an islet cell tumor that was releasing insulin and somatostatin. A fasting test suggested the over secretion of insulin. Moreover, this hypoglycemia was enhanced by the inhibitory effect of somatostatin on the secretion of insulin counter-regulatory hormones, such as glucagon, in addition to the autonomous secretion o...
